Verify the bank's notice and ask whether your case is no change, address-only change or another update.

  1. Verify the request through your bank's official app, site, branch or published contact.
  2. Read the requirements for your account/customer category and the change actually involved.
  3. Do not place identity-document images, passwords, PINs or OTPs in this sheet.

1. VERIFY THE REQUEST

Confirm the notice using the bank's published contact or service. Record the actual bank requirement and its date. A message containing an unfamiliar upload link is not enough to establish that the bank sent it.

Describe the facts accurately. A no-change statement should be used only if that is true. An address-only update, a legal name change, a new authorised signatory and a periodic review may need different processes.

Record document names and requirements rather than copying identity numbers or images into this sheet. Check that the information matches the account holder's correct name and capacity. If documents differ, ask the bank how the discrepancy should be resolved.

Ask which form and channel are permitted for this case, whether originals need inspection and how acknowledgment will be issued. Where a declaration route is permitted, use the bank's actual declaration and verification process.

6. SUBMISSION AND STATUS

After submission, record the reference and what the bank accepted. Check whether the KYC status was updated and whether a separate restriction remains. A successful upload, this completed sheet or an acknowledgment does not prove that the bank has approved every update.

Keep passwords, PINs, OTPs and document images outside this worksheet. Provide required records only through the verified bank process. This preparation sheet neither submits KYC nor replaces a bank-prescribed declaration.

Your next moves

  1. Use the bank's prescribed form or authenticated update route and supply documents only where required.
  2. Keep the bank's acknowledgment and check the updated status or any continuing restriction.

Real-world check

Private preparation only, not a KYC declaration or submission. Accepted documents, self-declaration, digital, branch or assisted routes depend on current bank instructions and customer facts. SBI's example is not a universal entitlement to its channel at every bank.
