If a nominee is a minor, ask for the actual form's permitted arrangement for receipt during minority. Do not insert a guessed adult recipient or claim authority over another holder's nomination. 4. ARRANGEMENT YOU INTEND [Chosen arrangement] Use only the arrangement permitted for the product. Current forms distinguish successive priority from simultaneous nomination where permitted. Do not treat the order of names as harmless if that order carries priority. For a simultaneous deposit arrangement, check that the stated shares cover the whole deposit as required by the form. 5. REQUIREMENTS CHECKED WITH THE BANK [Bank's actual form requirements] Read the current prescribed form fully. Record holder signatures, authority and any actual witness or other requirement. Do not assume a general affidavit, notarisation or this planning sheet is the required instrument. 6. QUESTIONS OR NEXT ACTION [Requested action] Ask the bank to explain an unclear field or existing nomination status before signing. A bank deposit nomination is separate from pension-arrears instructions, a will or the final determination of inheritance. After submission through the prescribed channel, keep the acknowledgment and verify the registered nomination. This sheet itself neither creates nor registers a nomination and should not be sent as a substitute for the bank's required form.
